Insurance Information
We accept most major insurance plans.
Appointments
Office visits to see our physicians are by appointment only. Please call one of our offices to make an appointment in advance. We request that new patients arrive 15 minutes prior to their initial visit to register. If possible, please download our registration forms (see below) and fill them out in advance so that we may expedite your visit.
If you are a pre-existing patient to our practice, but have changed your health insurance or residence, please alert the staff of these changes. You will be required to fill out a new Patient Registration form so that we may update your file. Please download the appropriate Patient Registration form from the Patient Registration Form Section below.
We require 24 hours notice if you are unable to keep your appointment so that we may use that time to see another patient in need of our services. Please note that a $50.00 cancellation will be billed to you for missed appointments if proper notification has not been given.
Specialist Referrals
If your health plan requires that you have a referral to see an orthopedic specialist it is the patient’s responsibility to bring their referral with them the day of their appointment. Failure to bring your referral will necessitate rescheduling your appointment. Per your health plan guidelines, we are not authorized to see any patient without an appropriate referral.
Fees & Payments
If we are a participating provider with your insurance plan, we will bill them directly. Should you have any questions regarding your bill, please contact our billing office at 301-604-3228.
Many health plans require patients to pay co-payments for office visits to specialists. Please understand that your health plan requires us to collect this payment as part of the contract that we have entered into with them. Per your health plan guidelines, we are required to collect all co-payments at the time that services are rendered. Failure to provide your co-payment will necessitate rescheduling your appointment. For your convenience we accept cash, personal checks, Visa and MasterCard.
Patient requests for our physicians to fill out forms require a fee of $30.00 per form. Example forms that we charge for include, but are not limited to, Family and Medical Leave Act, medical disability and work modification/release forms.
Prescriptions & Renewals
Please request all prescriptions and authorization for renewals during our normal office hours when your medical records are available for review. Prescription requests after hours or weekends will only be filled under extreme or emergent circumstances.
