Appointments
Office visits to see our physicians are by appointment only.
Please call one of our offices to make an appointment in advance. We request that new patients arrive 15 minutes prior to their initial visit to register.
If you are a pre-existing patient to our practice, but have changed your health insurance or residence, please alert the staff of these changes.
The practice requires a 24 hour notice if you are unable to keep your appointment so that we may use that time to see another patient in need of our services. Please note that a $50.00 cancellation will be billed to you for missed appointments if proper notification has not been given.
Please bring the following items to your appointment:
- Photo ID
- Insurance card
- Specialist referral, if required
- Co-payments
- Medical information and medication list
- Radiology studies such as X-rays and MRIs
- A parent or legal guardian must accompany patients under 18 years of age
