Office Fees and Payments

If we are a participating provider with your insurance plan, we will bill them directly. Should you have any questions regarding your bill, please contact our billing office at 301-604-3228.

Many health plans require patients to pay co-payments for office visits to specialists. Please understand that your health plan requires us to collect this payment as part of the contract that we have entered into with them. Per your health plan guidelines, we are required to collect all co-payments at the time that services are rendered. Failure to provide your co-payment will necessitate rescheduling your appointment. For your convenience we accept cash, personal checks, Visa and MasterCard. 

Patient requests for our physicians to fill out forms require a fee of $30.00 per form. Typical forms that we charge for include, but are not limited to, Family and Medical Leave Act, medical disability and work modification/release forms.
